Important Things to Remember for 2012-2013


Supplemental Insurances require employees to work a minimum of 30 hours per week to be eligible.

Supplemental benefit changes for this annual enrollment will become effective 2/01/2012. Elections requiring evidence of insurability may have a later effective date, if approved.

Flexible Spending Accounts –The NEW administrator for the Flex Accounts is National Benefit Services – NBS. NBS is the Third Party Administrator (TPA) for Burleson ISD for plan year January 1, 2012 through December 31, 2012. The maximum annual contribution for medical reimbursement is $300/month and for dependent care reimbursement it is $416.66/month.

Important Reminder: The same IRS125 Plan that allows you to participate in this plan also requires that you forfeit any unused money left in your account at the end of the Plan year or at your termination. This is the "USE IT OR LOSE IT" provision of the IRS Code. It is very important that you estimate your expenses carefully and conservatively. Eligible expenses must be incurred in the plan year of 1/1/2012 through 12/31/2012. Direct deposit is available. YOU MUST RE-ENROLL EVERY YEAR FOR THE FLEXIBLE SPENDING ACCOUNT. Benefits will not “roll-over” year to year.

FSA OTC CHANGE: Effective beginning on January 1,2011, over the counter medicines or drugs will no longer be eligible for reimbursement under an FSA without a doctor's prescription. Plan your contributions carefully! For additional information refer to the Benefits & Forms tab/reimbursement plan.

Flex Cards: If you currently participate in the Medical Reimbursement Account, you will receive a new NBS Flex Card. Your old “MGM” card will not work after 12/31/2011. The new NBS Flex card will be mailed to your home within 2 to 3 weeks after the beginning of the plan year. Be sure to update your address in the system. Cards cannot be delivered to an incorrect address!

SECTION 125 CAFETERIA PLAN RULE: All benefit elections will remain in effect and cannot be revoked or changed during the plan year unless you have one of the following changes in family status: Marriage, Divorce, Birth, Adoption, Death, Termination or change in employee or spouse’s employment (full-time to part-time/part-time to full-time status), Change in eligibility status of a dependent (attains maximum eligibility age / meets and/or ceases to meet full-time student status qualification), Loss or curtailment in health coverage of employee or spouse due to change in spouse’s employment and upon meeting a required eligibility period. NOTE: You must provide documentation from an independent third-party verifying that you have experienced the indicated event within 31 days of the event.

Dental - NEW! CIGNA Dental is the carrier for Burleson ISD’s 3 option dental plans effective 2/1/2012.

  1. OPTION 1: CIGNA’s HIGH PPO is an indemnity dental plan.
  2. OPTION 2: CIGNA’s Low PPO option that does not include orthodontia for children.
  3. OPTION 3: The CIGNA’s DHMO is the fee schedule dental plan.

Unum Disability - During the annual enrollment period, employees can increase or add disability coverage without providing evidence of insurability! New coverage amounts or changes are subject to pre-existing condition limitations. See plan information under the Benefits & Forms tab for details on plan options A and B, and benefit changes.

Block Vision 150 Plan - is the vision provider for this year. Click on the Benefits & Forms tab /Vision for plan information, plan options and rates and to locate a provider.

American Public Life (APL) - is the cancer provider for Burleson ISD this year. For plan and rate information click on the Benefits & Forms tab /cancer.

UNUM - Voluntary Life, Dependent Term Life and AD&D is being offered through UNUM. Newly hired employees may apply for up to 7x salary to a maximum of $200,000 for employees, $50,000 for spouse and $10,000 for children, “Guarantee Issue” (no medical questions asked). Existing employees may apply for coverage subject to medical underwriting and health questions. All applications for new or coverage will be online. No paper forms. See plan information under the Benefits & Forms tab for details on plan options.

Current product offerings to Burleson ISD employees include: CIGNA’s 3 tier dental plans, Block Vision plan, Unum Educator Select Disability Plan, American Public Life “APL” Cancer Plan, UNUM Term Life AD&D, and Flexible Spending Accounts.
