IMPORTANT: ALL HEALTHCARE REIMBURSEMENT PARTICIPANTS WILL RECEIVE A NEW FLEX CARD FOR THE PLAN YEAR BEGINNING 1/1/2010. “BENNY” FLEX CARDS ARE NO LONGER ACTIVE AS OF THE LAST DAY OF THE PRIOR PLAN YEAR, 12/31/2009. CLICK ON BENEFITS AND FORMS/REIMBURSEMENT ACCOUNTS FOR MORE INFORMATION.

Important Things to Know for 2010

  1. Burleson ISD Cafeteria plan year is January 1, 2010 through December 31, 2010. NEW! This year the Medical Reimbursement amount has increased to $3,600 per year.

    All changes and new products for supplemental insurance this plan year will be effective February 1, 2010.

  2. Mass Group Marketing will continue to be the Third Party Administrator (TPA) for the Cafeteria plans for Burleson ISD for the plan year January 1, 2010 through December 31, 2010. NEW! the “MGM Flex Card” for all participants of the medical Flexible Spending Accounts will replace the Benny MasterCard. The fee for the flex card is employer paid. See plan information under the Benefits & Forms tab for details on plan options.


  3. Burleson ISD offers a 3 option dental plan. NEW! The dental provider this year will be OraQuest Dental.
    • OPTION 1: OraQuest PPO is indemnity dental plan offered this year.
    • OPTION 2: OraQuest “Network PPO” option for employees that use “in network providers”.
    • OPTION 3: The OraQuest DHMO is the fee schedule dental plan offered this year. For information and the rates, see plan information under the Benefits & Forms tab for details on plan options and to locate a provider.

  4. The Unum Disability plan this year is the Educator Select Plan. During the annual enrollment period, employees can increase or add disability coverage without providing evidence of insurability! New coverage amounts or changes are subject to pre-existing condition limitations. See plan information under the Benefits & Forms tab for details on plan options and benefit changes.


  5. NEW! Block Vision 150 Plan is the vision provider for this year. Click on the Benefits & Forms tab /Vision for plan information, plan options and rates and to locate a provider.


  6. NEW! American Public Life (APL) is the cancer provider for Burleson ISD this year effective 02/01/2010. ALL Employees must move to the new APL Plan. Only persons currently in treatment or who have had treatment in the past 12 months can continue to payroll deduct prior cancer plans. For plan and rate information click on the Benefits & Forms tab /cancer.


  7. NEW! Voluntary Life, Dependent Term Life and AD&D is being offered through UNUM. Existing coverage through Mutual of Omaha will automatically be moved to UNUM. Only this year, you may apply for up to 7x salary to a maximum of $200,000 for employees, $50,000 for spouse and $10,000 for children, “Guarantee Issue” (no medical questions asked). All applications for new coverage will be online. No paper forms. See plan information under the Benefits & Forms tab for details on plan options.


  8. Current product offerings to Burleson ISD employees include: OraQuest 3 tier dental plans, Block Vision plan, Unum Educator Select Disability Plan, American Public Life Cancer Plan “APL”, UNUM Term Life AD&D, and Flexible Spending Accounts.
